Registration & Fees
- IDS member rate (physicians & non-physician staff) -- $25 each
- Resident/fellows -- No charge (but registration still required)
- Not a member of IDS - Physicians or non-physician staff -- $50 each

Other information:
- Cancellations – Refunds, minus a $10 processing charge, may be requested for cancellations received at the IDS administrative office no later than 5 p.m. on January 25th.
- Substitutions – If a person registered is unable to attend, you may substitute somebody else from your office. Please contact the IDS office if you plan to make a substitution.
- Confirmations – All registrants will receive a confirmation and site map by regular mail, fax or email.
