  • IDS member rate (physicians & non-physician staff) -- $75 each
  • Resident/fellows -- No charge (but registration still required)
  • Not a member of IDS - Physicians and non-physician staff* -- $150 each

Other information:

  • Cancellations – Refunds, minus a $15 processing charge, may be requested for cancellations received at the IDS administrative office no later than 5 p.m. on January 22.
  • Substitutions – If a person registered is unable to attend, you may substitute somebody else from your office. Please contact the IDS office if you plan to make a substitution.
  • Confirmations – All registrants will receive a confirmation and site map by regular mail or fax.
